A new stoma service has been launched to benefit Nantwich patients.
The Specialist Nursing, Assessment and Prescription Service (SNAPS), run by Central Cheshire Integrated Care Partnership, will provide local stoma patients with direct access to specialist support and a prescribing hub for stoma products.
It means patients no longer have to go through their GPs and can access SNAPS to get prescriptions, general support and expert advice from the Stoma Care Specialist team.
The programme was launched this week for patients in Nantwich and Winsford.
The second phase will be rolled out on Monday August 5 for patients in Crewe, Northwich, Sandbach, Middlewich, Alsager, Scholar Green and Haslington.
SNAPS also delivers care across primary, community and hospital settings, providing ongoing support for stoma education and management, review following hospital discharge, and home visits to patients who may need them.
Julie Woodcock, Stoma Team Leader, said: “This is an exciting project that will ensure that all patients have easy access to specialist care and an effective prescription service.”
Tony Mayer, associate director for CCICP, added: “The team has worked tirelessly to create a much-improved service for patients with a stoma.
“Some patients, who have already had support and reviews from the specialist nurses, confirm that the new arrangements make a huge difference to not only their clinical care, but have also the improved social opportunities that sometimes impact people with a stoma.”
